Order 302n dated April 12, 2011 contains Appendix No. 3 - The procedure for conducting primary and periodic medical examinations.
The norms of this Appendix to Order 302n of the Ministry of Health and Social Development, in addition to the procedure for conducting medical examinations, regulate the requirements for their execution. According to these requirements, a medical institution licensed to conduct occupational medical examinations must have the following records for an employee sent for a primary medical examination:
- medical record of an outpatient (clause 10.1 of section II of Appendix No. 3)
- The card reflects detailed information about the results of the examination - laboratory and instrumental studies, doctors' opinions, and the conclusions of the commission. The card is stored in the medical institution;
- health passport (clause 10.2 of section II of Appendix No. 3)
- The document briefly duplicates information about the employee undergoing medical specialists and the necessary studies. According to Order 302n, an employee’s health passport is issued by a medical institution directly for the employee - it is handed over to him upon completion of the medical examination.
Who is responsible for issuing the passport?
Filling out the title page indicating the date is carried out at the registry of the state clinic. It must have a license to provide this type of service to the public.
If an erroneous entry was made during the examination, corrections and deletions are not allowed. In the appropriate column, the medical professional must make a note “the entry is erroneous” indicating the date and time the changes were made. It must be certified by 2 signatures of responsible persons and the seal of the institution.
Order 302n strictly regulates only the list of data that is indicated on the title page. There are no strict rules for filling out the remaining sheets, so the form is free.

The speed of passing the commission depends on the list of tests and specialists who need to be visited. A sample list includes:
- measurement of height, weight;
- general analysis of urine and blood;
- blood pressure measurement;
- biochemical study of biological fluid;
- blood test for HIV, parenteral hepatitis;
- after 40 years, cholesterol test;
