What documents are drawn up during a periodic medical examination, who pays for it?

Unlike the initial medical examination, which an employee undergoes before concluding an employment agreement with the employer, when undergoing a periodic medical examination, a certificate drawn up in form No. 086/u or No. 001-GS/u is not required.

Moreover, if the period of such an inspection exceeds 10 working days allotted for making a decision on professional suitability, then it can be extended up to 30 working days, namely until the results of the inspections are received. The period cannot be extended by more than 30 days.

The final decision of the commission is formalized in the form of a protocol, which serves as the basis for a medical report on unsuitability or suitability for certain types of work.

The protocol is drawn up in the form specified in Appendix 2 of Order of the Ministry of Health No. 282n.

The medical report is drawn up in 2 copies within 3 working days from the date of the decision of the medical commission. One copy of the conclusion is given to the employee so that he can present it to his employer, and the employee signs in the journal for registering medical reports.

Once a year, fluorography, consultation with a therapist and laboratory tests for the presence of helminths are prescribed.

• Children's preschool, school and secondary vocational medical institutions - examinations for the presence of STDs, infectious diseases and bacteriological studies are carried out up to 4 times a year. A general therapeutic commission with fluorography and laboratory tests is required once a year.
• Pharmacies and non-food trade - once a year, examination by a dermatovenerologist, therapist, fluorography and laboratory tests are indicated.
• Communal services for the population and swimming pools - they are examined for the presence of STDs 2 times a year and undergo a standard medical examination 1 time a year.

Features of the medical examination certificate

Regular inspection was approved by Order 302 dated April 12, 2011.
According to this official document, the inspection can be preliminary (i.e. upon entry to the place of work), periodic or extraordinary. Depending on this, the form of the certificate that is issued based on the results of the examination will also change. Each employee receives an individual health passport, which is issued in form No. 004-P/U. It includes general and effective parts. The first contains information about the employee, position held, working conditions and length of service. The results part includes information about the appointment of a therapist, surgeon, ENT specialist, ophthalmologist, narcologist and psychiatrist. Blood tests and fluorography are also provided.

The health passport must have doctors' signatures and seals, otherwise it is considered invalid.

Appendix No. 1. Procedure for conducting examination of professional suitability

Appendix No. 1 to the order of the Ministry of Health of the Russian Federation dated May 5, 2020 No. 282n

1. This procedure determines the rules for conducting an examination of professional suitability in order to determine whether the health status of an employee (person entering work) (hereinafter referred to as the employee) is suitable for performing certain types of work.

2. An examination of professional suitability is carried out based on the results of preliminary medical examinations and periodic medical examinations (hereinafter referred to as the mandatory medical examination) in relation to employees who, during a mandatory medical examination, have had medical contraindications to certain types of work.

3. The examination of professional suitability is carried out in a medical organization or a structural unit of a medical organization or other organization, regardless of the organizational and legal form, that has a license to carry out medical activities for the examination of professional suitability (hereinafter referred to as the medical organization). ________________ Decree of the Government of the Russian Federation of April 16, 2012 N 291 “On licensing of medical activities (with the exception of the specified activities carried out by medical organizations and other organizations included in the private healthcare system in the territory of the innovative)” (Collected Legislation of the Russian Federation, 2012, N 17, Art. 1965; N 37, Art. 5002; 2013, N 3, Art. 207; N 16, Art. 1970).

4. To conduct an examination of professional suitability in a medical organization, a permanent medical commission is formed. ________________ Order of the Ministry of Health and Social Development of the Russian Federation dated May 5, 2012 N 502n “On approval of the procedure for the creation and activities of a medical commission of a medical organization” (registered by the Ministry of Justice of the Russian Federation on June 9, 2012, registration N 24516) as amended by order of the Ministry of Health Russian Federation dated December 2, 2013 N 886н (registered by the Ministry of Justice of the Russian Federation on December 23, 2013, registration N 30714).

5. The head of the medical organization (deputy head of the medical organization, head of the structural unit of the medical organization) who meets the Qualification requirements for medical pharmaceutical workers with higher education in the field of training “Healthcare and Medical Sciences”, approved by order of the Ministry of Health of the Russian Federation dated 8 October 2020 N 707n (registered by the Ministry of Justice of the Russian Federation on October 23, 2020, registration N 39438) in the specialty “occupational pathology”.

6. To conduct an examination of professional suitability, the employee submits to the medical organization: an identification document; a referral issued by a medical organization that conducted a mandatory medical examination, during which medical contraindications to certain types of work were identified; medical certificate based on the results of a mandatory medical examination issued to the employee.

7. The medical commission, within 10 working days from the date of receipt of the documents specified in paragraph 6 of this Procedure by the medical organization, reviews these documents.

8. The medical commission of a medical organization, based on the results of a mandatory medical examination, makes one of the following decisions to recognize the employee: fit for health reasons to perform certain types of work; temporarily unfit for health reasons to perform certain types of work; permanently unfit for health reasons to perform certain types of work. If a decision is made on temporary unfitness for health reasons to perform certain types of work, the rationale for this decision and the period of temporary unfitness with recommendations for additional studies (laboratory, instrumental studies) and (or) appropriate treatment are indicated. The final decision is made by the commission after presenting the results of the studies and (or) treatment performed.

9. The decision of the medical commission is drawn up in the form of a protocol.

10. If it is impossible, on the basis of the documents specified in paragraph 6 of this Procedure, to determine the suitability of the employee’s health status for performing certain types of work, the medical commission makes a decision to order examinations (research) by medical specialists for the employee.

11. If, during the examination of professional suitability, the medical commission has appointed examinations by medical specialists (studies), the duration of which exceeds the period specified in paragraph 7 of this Procedure, the period for conducting the examination of professional suitability is extended until the results of these examinations (studies) are received. , but not more than 30 working days. This decision of the medical commission, containing a reasoned justification, is reflected in the protocol of the medical commission.

12. Based on the protocol of the medical commission, the medical worker authorized by the head of the medical organization draws up a medical report on suitability or unsuitability to perform certain types of work (hereinafter referred to as the Medical Report) in the form provided for in Appendix No. 2 to this order.

13. A medical report is issued within 3 working days from the date of one of the decisions of the medical commission specified in paragraph 8 of this Procedure.

14. The medical report is drawn up in two copies, one of which: is given to the employee for presentation to the employer, about which the employee signs in the journal of registration of Medical reports; pasted into the employee’s medical documentation issued by a medical organization and stored for 50 years.

Medical examination and work

These include construction and high-rise services, consumer services, transport work, chemical production and much more. 2. Biochemical blood test. This is a study that allows you to determine the correctness of the biochemical processes occurring in the body.

3. Fluorography. Chest examination is aimed at detecting tuberculosis in the early stages and prompt treatment.

4. Electrocardiogram. It will allow you to determine deviations in heart rhythm and promptly identify the presence of pathologies.

5. Women must undergo an examination by a gynecologist, during which smears are taken to identify pathogenic microflora.

Certificate 302 n does not have a clear established form, but it must reflect the basic data of the patient and contain the details of the medical institution. This is the number, signatures and personal stamps of specialists, the seal of the health care facility and the date of issue. Accredited private medical organizations have the right to issue such a document if they can provide a medical examination by experienced specialists, taking into account legal requirements.

The medical report is drawn up in two copies within 3 working days from the date of the relevant decision of the medical commission. One copy of the medical report is given to the employee for presentation to the employer, which the employee signs in the journal for registering medical reports.

The list of contingents is approved once, until any changes occur at the enterprise (new jobs, improvement or deterioration of working conditions, reorganization). The finished document is sent to Rospotrebnadzor.

Name lists of persons are developed annually two months before the agreed date of the medical examination.

Important It must indicate the working experience precisely in the conditions of the declared production factor. It should be noted that a periodic medical examination is carried out at least once every 2 years in a medical institution and once every 5 years in an occupational pathology center. The lists are compiled separately.

The form of the certificate, the procedure for its preparation and submission to the insurance organization are determined by Order of the Russian Ministry of Defense dated December 24, 2015 No. 833.

Despite the expansion of the rights of medical workers when prescribing and prescribing NS and PV, many organizational problems remain unresolved. So patients suffering from chronic pain syndrome cannot always receive state-guaranteed analgesic therapy of adequate quality.

Until these problems are resolved, the heads of medical organizations and their deputies for medical work need to:

• study the approved changes in federal regulatory documents in the field of circulation of NS and PV for medical purposes and bring their content to the attention of doctors;
• take into account federal regulations when drawing up local orders and bring these orders to the attention of specialists, including employees of pharmacy organizations;
• make proposals to optimize the provision of narcotic drugs to patients to regional health authorities;
• interact more actively with pharmacy organizations when drawing up applications for the necessary NS and PT;
• regularly train doctors on adequate pain management and modern principles of pain management;
• direct all efforts to explanatory work on the timely receipt (return) of the social package by patients. In this case, at least the main organizational and financial flow for providing patients with narcotic drugs will be formed.

A referral for a medical examination on the employer’s letterhead is given to the specialist applying for a job in person against signature. Refusal to receive a referral or undergo a medical examination means the applicant’s failure to fulfill his obligations, as a result of which the employment contract (if it was signed) is canceled.

The medical examination is considered completed if the specialist has visited all doctors, and all planned laboratory and diagnostic tests have been carried out. Based on the results of the examination, the person applying for work is given a conclusion.

The second copy of the report remains in the medical institution and is attached to the outpatient medical record, which is created immediately after presentation of the referral for a medical examination.

The referral form for a preliminary or periodic medical examination does not have an approved form. However, there is a list of required details (clause 8 of Appendix No. 3 to the order of the Ministry of Health and Social Development dated April 12, 2011 No. 302n):

• employer name;
• form of ownership and type of activity of the employer according to OKVED;
• name of the medical institution, its registration and contact information;
• type of medical examination;
• Full name of the employee and his date of birth;
• name of the employer's department in which the employee works (will work);
• name of position, type of work, profession;
• factors harmful to health at work.

An authorized employee of the employer fills out the form for a referral for a medical examination.

Purpose of the examination

According to Article 63 of the Law of November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in Russia”, an examination of professional suitability is a determination of the compliance of an employee’s health with the ability to perform certain types of work.

Rules for issuing a certificate

A medical examination is carried out before the end of treatment. Persons who have received severe injuries, the treatment of which should take more than two months, undergo a medical examination after a final diagnosis has been established, i.e., before the completion of the full course of treatment.

The IHC issues a certificate only in relation to injuries listed in the list of Resolution No. 855.

In case of combined or combined injury, the severity should be determined by the most severe injury.

In the certificate of severity of injury, the VVK indicates the final diagnosis, and in cases established by the list of Resolution No. 855, the state of function of the damaged organs or organ systems. The state of the function determines the VVC upon completion of the main course of treatment.

Following these rules will eliminate cases of unjustified issuance of a certificate.

In addition to the medical aspect, the social aspect is important in the work of the VVK. Eliminating errors in certificates of severity of injuries will increase the level of social protection of military personnel.

Application

Reducing the production of sterile drugs

Order No. 751n significantly reduced the number of items of sterile preparations to be manufactured (compared to Order No. 214 of the Russian Ministry of Health dated July 16, 1997).

Example. The list of solutions for injections and infusions has been reduced from 65 to 50; eye drops, irrigation solutions, concentrates for making eye drops - from 66 to 37; medicines for newborns - from 43 to 34 positions.

The reduction in production is due to a number of reasons.

First reason.

Article 56 of the Federal Law of April 12, 2010 No. 61-FZ “On the Circulation of Medicines” prohibited pharmacy organizations from producing medicines registered in the Russian Federation. Thus, the production of 0.9% sodium chloride solution and 5.0% glucose solution in 200 and 400 ml packaging, which made up a significant share of the total volume of pharmaceutical products, was banned.

The second reason.

Some extemporaneous drugs have been transferred to industrial production.

Example. The manufacturer stopped supplying the iodopirone substance to industrial pharmacies and independently began producing a 1.0% iodopirone solution. Pharmacy-made nasal drops with protargol for children have been replaced by the ready-made drug “Sialor”.

Third reason.

It is unacceptable to use pharmaceutical substances whose quality does not meet the requirements of the standards, or substances that are not registered in the Russian Federation. Pharmacies do not always have the opportunity to purchase the necessary substances of the required quality and with the appropriate accompanying documents.

Example. The registered substance potassium iodide no longer meets the requirements of the State Pharmacopoeia in terms of microbiological purity and suitable raw materials for the manufacture of sterile preparations.
