    Features of the medical examination certificate

    Appendix No. 1. Procedure for conducting examination of professional suitability

    Appendix No. 1 to the order of the Ministry of Health of the Russian Federation dated May 5, 2020 No. 282n

    1. This procedure determines the rules for conducting an examination of professional suitability in order to determine whether the health status of an employee (person entering work) (hereinafter referred to as the employee) is suitable for performing certain types of work.

    2. An examination of professional suitability is carried out based on the results of preliminary medical examinations and periodic medical examinations (hereinafter referred to as the mandatory medical examination) in relation to employees who, during a mandatory medical examination, have had medical contraindications to certain types of work.

    3. The examination of professional suitability is carried out in a medical organization or a structural unit of a medical organization or other organization, regardless of the organizational and legal form, that has a license to carry out medical activities for the examination of professional suitability (hereinafter referred to as the medical organization). ________________ Decree of the Government of the Russian Federation of April 16, 2012 N 291 “On licensing of medical activities (with the exception of the specified activities carried out by medical organizations and other organizations included in the private healthcare system in the territory of the innovative)” (Collected Legislation of the Russian Federation, 2012, N 17, Art. 1965; N 37, Art. 5002; 2013, N 3, Art. 207; N 16, Art. 1970).

    4. To conduct an examination of professional suitability in a medical organization, a permanent medical commission is formed. ________________ Order of the Ministry of Health and Social Development of the Russian Federation dated May 5, 2012 N 502n “On approval of the procedure for the creation and activities of a medical commission of a medical organization” (registered by the Ministry of Justice of the Russian Federation on June 9, 2012, registration N 24516) as amended by order of the Ministry of Health Russian Federation dated December 2, 2013 N 886н (registered by the Ministry of Justice of the Russian Federation on December 23, 2013, registration N 30714).

    5. The head of the medical organization (deputy head of the medical organization, head of the structural unit of the medical organization) who meets the Qualification requirements for medical pharmaceutical workers with higher education in the field of training “Healthcare and Medical Sciences”, approved by order of the Ministry of Health of the Russian Federation dated 8 October 2020 N 707n (registered by the Ministry of Justice of the Russian Federation on October 23, 2020, registration N 39438) in the specialty “occupational pathology”.

    6. To conduct an examination of professional suitability, the employee submits to the medical organization: an identification document; a referral issued by a medical organization that conducted a mandatory medical examination, during which medical contraindications to certain types of work were identified; medical certificate based on the results of a mandatory medical examination issued to the employee.

    7. The medical commission, within 10 working days from the date of receipt of the documents specified in paragraph 6 of this Procedure by the medical organization, reviews these documents.

    8. The medical commission of a medical organization, based on the results of a mandatory medical examination, makes one of the following decisions to recognize the employee: fit for health reasons to perform certain types of work; temporarily unfit for health reasons to perform certain types of work; permanently unfit for health reasons to perform certain types of work. If a decision is made on temporary unfitness for health reasons to perform certain types of work, the rationale for this decision and the period of temporary unfitness with recommendations for additional studies (laboratory, instrumental studies) and (or) appropriate treatment are indicated. The final decision is made by the commission after presenting the results of the studies and (or) treatment performed.

    9. The decision of the medical commission is drawn up in the form of a protocol.

    10. If it is impossible, on the basis of the documents specified in paragraph 6 of this Procedure, to determine the suitability of the employee’s health status for performing certain types of work, the medical commission makes a decision to order examinations (research) by medical specialists for the employee.

    11. If, during the examination of professional suitability, the medical commission has appointed examinations by medical specialists (studies), the duration of which exceeds the period specified in paragraph 7 of this Procedure, the period for conducting the examination of professional suitability is extended until the results of these examinations (studies) are received. , but not more than 30 working days. This decision of the medical commission, containing a reasoned justification, is reflected in the protocol of the medical commission.

    12. Based on the protocol of the medical commission, the medical worker authorized by the head of the medical organization draws up a medical report on suitability or unsuitability to perform certain types of work (hereinafter referred to as the Medical Report) in the form provided for in Appendix No. 2 to this order.

    13. A medical report is issued within 3 working days from the date of one of the decisions of the medical commission specified in paragraph 8 of this Procedure.

    14. The medical report is drawn up in two copies, one of which: is given to the employee for presentation to the employer, about which the employee signs in the journal of registration of Medical reports; pasted into the employee’s medical documentation issued by a medical organization and stored for 50 years.

    The referral form for a preliminary or periodic medical examination does not have an approved form. However, there is a list of required details (clause 8 of Appendix No. 3 to the order of the Ministry of Health and Social Development dated April 12, 2011 No. 302n):

    • name of the medical institution, its registration and contact information;
    • type of medical examination;
    • Full name of the employee and his date of birth;
    • name of the employer's department in which the employee works (will work);

    • factors harmful to health at work.

    According to Article 63 of the Law of November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in Russia”, an examination of professional suitability is a determination of the compliance of an employee’s health with the ability to perform certain types of work.

    Rules for issuing a certificate

    A medical examination is carried out before the end of treatment. Persons who have received severe injuries, the treatment of which should take more than two months, undergo a medical examination after a final diagnosis has been established, i.e., before the completion of the full course of treatment.

    The IHC issues a certificate only in relation to injuries listed in the list of Resolution No. 855.

    In case of combined or combined injury, the severity should be determined by the most severe injury.

    In addition to the medical aspect, the social aspect is important in the work of the VVK. Eliminating errors in certificates of severity of injuries will increase the level of social protection of military personnel.

    Application

    Reducing the production of sterile drugs

    Order No. 751n significantly reduced the number of items of sterile preparations to be manufactured (compared to Order No. 214 of the Russian Ministry of Health dated July 16, 1997).

    Example. The list of solutions for injections and infusions has been reduced from 65 to 50; eye drops, irrigation solutions, concentrates for making eye drops - from 66 to 37; medicines for newborns - from 43 to 34 positions.

    Requirements for sterilization regimes for medicinal products are given in Appendix 15.

    The reduction in production is due to a number of reasons.

    First reason.

    Article 56 of the Federal Law of April 12, 2010 No. 61-FZ “On the Circulation of Medicines” prohibited pharmacy organizations from producing medicines registered in the Russian Federation. Thus, the production of 0.9% sodium chloride solution and 5.0% glucose solution in 200 and 400 ml packaging, which made up a significant share of the total volume of pharmaceutical products, was banned.

    The second reason.

    Some extemporaneous drugs have been transferred to industrial production.

    Example. The manufacturer stopped supplying the iodopirone substance to industrial pharmacies and independently began producing a 1.0% iodopirone solution. Pharmacy-made nasal drops with protargol for children have been replaced by the ready-made drug “Sialor”.

    Third reason.

    It is unacceptable to use pharmaceutical substances whose quality does not meet the requirements of the standards, or substances that are not registered in the Russian Federation. Pharmacies do not always have the opportunity to purchase the necessary substances of the required quality and with the appropriate accompanying documents.

    Example. The registered substance potassium iodide no longer meets the requirements of the State Pharmacopoeia in terms of microbiological purity and suitable raw materials for the manufacture of sterile preparations. As a result, pharmacies stopped producing eye drops. The situation is similar with a 10.0% glycerin solution for intravenous administration, which was in high demand in intensive care units.

    Doctors' practice has included prescribing captopril, digoxin, and L-thyroxine to newborns and young children. Their substances are not registered, and the need for them is very small. Meanwhile, registered substances, for example phenobarbital, are produced in a minimum packaging of 1 kg, which even for large pharmacies amounts to a 2-3-year supply and sharply increases the costs of the pharmacy.

    The ban on the production of certain concentrations and packaging volumes of sterile solutions that differ from those produced by industry causes problems in medical organizations.

    Not only the Unified State Exam

    According to the certification procedure, a teacher of a school included in the top 399 who has contributed to the school’s achievement of such positions, by decision of the administration, can be certified without uploading all learning results and without considering them at a meeting of the City Certification Commission. Moreover, not just the actual results of students studying with a given teacher are taken into account, but the dynamics: improvement in the performance of both weak and strong students.

    But what about a teacher working in a school where there is a difficult group of children who cannot pass the Unified State Exam with high scores or become winners of Olympiads?

    “Moscow provides many opportunities for a teacher to prove himself and pass painless certification,” says the chairman of the subject association of foreign language teachers of the city of Moscow Ekaterina Morozova . – For example, if a teacher has been trained to become an expert of the subject commission of the Unified State Examination (checks the work of graduates) and has worked as one for at least 3 years, then certification is carried out automatically. If a teacher wins a professional competition (there is an approved list of them) and actively works in the Moscow Electronic School system, then he can also pass certification according to a simplified scheme.”
