Forms and work order forms

Individual entrepreneurs and companies, when providing various services, have the right to carry out with the population:

  • payments by payment cards without the use of cash register equipment;
  • and (or) cash payments.

In such cases, the legislation provides for the fulfillment of a mandatory condition. Service providers are required to issue customer-citizens a document equivalent to a cash receipt. It must be issued on a strict reporting form.

The regulations approved by Decree of the Government of the Russian Federation dated May 6, 2008 N 359 define a list of mandatory details of such a document. These include:

  • document title, 6-digit number and series;
  • the name of the company and its legal form, and for an individual entrepreneur - his last name, first name and patronymic;
  • the location of the permanent executive body of the organization (if there is none, then another body or person who has the right to act on behalf of the company without a power of attorney);
  • individual taxpayer number (TIN);
  • description (type) of service;
  • amount of payment;
  • cost of work (service) in monetary terms;
  • date of drawing up the document and carrying out the calculation;
  • the position of the person who is responsible for the transaction, his name, surname, patronymic, personal signature and seal of the company (or individual entrepreneur);
  • other details that contain the characteristics and specifics of the service provided.

In practice, such a document is called a work order (or work order, task order).

In 1995, the Ministry of Finance of the Russian Federation approved forms of work orders for the performance of certain types of household services (Letter dated April 20, 1995 N 16-00-30-33). These forms can also be used in 2020 if they contain all the required details listed above.

The classification of services as household services is determined by the All-Russian Classifier of Services to the Population (OKUN). Therefore, the use of a specific form of the form is tied to OKUN codes.

Rules for filling out a work order

Regulations, approved Decree of the Government of the Russian Federation dated 05/06/08 No. 359 defines a list of mandatory details of such a document. The work order form for work must contain:

  • title of the document, its number and series;
  • name of the enterprise and its legal form/full name. for individual entrepreneurs;
  • location of the permanent executive body of the legal entity (in the absence of a permanent executive body of the legal entity - another body or person entitled to act on behalf of the legal entity without a power of attorney);
  • TIN;
  • type of service provided;
  • amount of payment;
  • cost of the service in monetary terms;
  • date of drawing up the document and carrying out the calculation;
  • Full name, position and signature of the person responsible for the transaction and seal of the organization or individual entrepreneur;
  • other details that contain the characteristics and specifics of the service provided.

Instructions for filling out the work permit for hot work

  • In the upper right part of the document the full name of the enterprise whose employees carry out hot work, as well as the structural unit to which they belong, is indicated.
  • In the upper left part, information about the head of the enterprise is entered, as well as the date of registration of the permit. Here, after the final completion of the document, the director will have to sign.
  • Below in the center of the line is the name of the document.
  • Then, in separate paragraphs, it is written down to whom the permit was issued (i.e., the person responsible for carrying out the work), for what kind of work, as well as information about the object (here you must indicate the name of the object and its actual address).
  • The fourth paragraph contains information about the performers: here you need to indicate their profession, qualifications, and the date of the last training completed. Each participant in the work must sign opposite his/her name.

The following paragraphs of the permit relate directly to the hot work being carried out.

  • First you need to indicate their start and end times (hours, minutes), then the fire safety measures taken (in detail).
  • Further, it should be noted that hot work, as well as measures taken to ensure fire safety, have been agreed upon with the responsible services of the facility - here it is necessary to indicate a specific person, his position, surname, initials, and the date of approval.
  • Finally, in the last paragraph of this part of the work permit, we similarly note the employee responsible for preparing the workplace for the implementation of assigned labor tasks.

According to the rules, a work permit must be issued for all types of work separately for one day shift.

If the work is not completed within the period established by the permit, then in paragraph 9 you need to make an entry indicating until what time they are extended (but not more than for one work shift). The next paragraph should include a note about agreeing to extend the permit for hot work.

If changes are made to the composition of the performing team (some workers are replaced by others, or someone is simply excluded or added), this should be noted in a special table.

After all the work is completed, the fact of completion of the work, the date and time are recorded, and the signature of the person in charge is affixed. After final registration, the work permit is submitted to the head of the enterprise for approval and signature.

How to prepare technical specifications for work

Technical specifications are understood as specialized documentation containing the customer’s main requirements for the procurement items, which defines a list of conditions under which the purchase will be carried out and what work is to be performed.

Despite the fact that the legislation does not provide for the mandatory preparation of technical specifications for the performance of work and other services, this form of documentation is widespread, since thanks to the technical specifications, labor and time resources are saved at the following stages:

  • Planning stage;
  • Drawing up final documentation for the upcoming purchase, notices, project agreements;
  • The stage of direct execution of the terms of the contract.

By drawing up a technical specification (hereinafter referred to as TOR) before posting a notice of a planned purchase, an optimal calculation of the time required to complete the work or provide goods and select qualified specialists is achieved.

Also, a properly compiled technical specification allows you to specify the procurement object itself as much as possible, describing it as clearly and in detail as possible.

Based on the previously prepared technical specifications, a final assessment of the compliance of the procurement result with the originally stated characteristics is carried out.

What goals are being achieved?

Based on the information contained in this document, it becomes possible:

  • Formation of a procurement plan and project;
  • Determining the cost of the contract, both initial and maximum possible;
  • Drawing up a notice of procurement;
  • Formation of a schedule for fulfilling the terms of the contract;
  • Preparation of basic documentation, including draft contracts;
  • Evaluation of proposals received from those wishing to participate in the procurement;
  • Conclusion of a contract and monitoring its implementation.

How to create a form

Approximate plan of technical specifications for the execution of work

As a rule, the terms of reference are drawn up by the contract manager in tandem with specialists from the legal department, after which the document is certified by a person authorized to make decisions related to procurement. It is advisable to sign the document no more than 10 days before the date of generation of the notice and other related procurement documents. The terms of reference include:

  • Basic information about the planned purchase;
  • General information about the procurement object;
  • Requirements for performers;
  • What conditions must be observed when executing the contract;
  • Information about available applications.

When drawing up technical specifications, it should be taken into account that, according to the technical specifications, the risks and burdens of suppliers and performers should not exceed those assumed by the customer, otherwise this may lead to an increase in the cost of the contract, or to refusal to submit applications for participation.

For construction and installation work

The terms of reference for construction and installation work must contain the necessary criteria according to which the required work must be carried out. When drawing up the document you should indicate:

  • The auction object itself. What kind of work must be performed in accordance with the future contract;
  • Location address. The exact location of the objects where construction and installation work is required;
  • Conditions of work. This paragraph, as a rule, lists the nature of the soil, engineering and geological characteristics, for example, the level of groundwater depth and other characteristics that are significant during future construction;
  • The nature of the construction and installation work is indicated - whether it will be new construction or the work will be carried out on an already constructed facility;
  • Method of implementation, for example, in a row;
  • The next paragraph contains information about the availability of design estimates and who compiled it;
  • Technical and economic characteristics of the construction site;
  • The next paragraph describes the functions that the customer of construction and installation work assumes, including accounting, monitoring the progress of construction at all stages, organizing work and providing permission to carry out construction and installation work;
  • Requirements for the contractor with a list of works to be performed by the contractor;
  • Stages of construction and deadlines for completing a certain volume according to the distribution into stages;
  • Organizational requirements, for example, the need to comply with the work performed to the requirements of GOST and current SNiPs;
  • The final point indicates the time frame within which construction and installation work must be completed in full.

For electrical installation work

When drawing up technical specifications for electrical installation work, the same principles apply as in the previous example. The following information is indicated by item:

  • Place of work;
  • Deadlines;
  • A brief description of the required work is given;
  • Requirements for the performer.

Thus, these may be requirements for the provision of information about admission to certain types of work, the availability of technical resources for their implementation, the provision of confirmation of the qualification level of the workers of the participant applying for participation in the auction.

To perform work under 44-FZ

According to the requirements of Federal Law No. 44-FZ, the customer must be guided by uniform requirements regarding the description of the procurement object when preparing documents, regardless of the methods of actual execution of the contract. When preparing the technical specifications, the customer must be guided by the following directives:

  1. When describing auction objects, one should focus on the criteria of objectivity;
  2. The functionality, technical and operational characteristics of the procurement object must be included in the description if necessary;
  3. The terms of reference should be neutral in nature, without containing too many excessive requirements in order to limit the number of potential participants.

Customers are required to rely on the provisions of Federal Law No. 44-FZ “On the contract system in the field of procurement of goods, works, services”, according to the requirements of which, the selection of a contractor or supplier is carried out according to the strict rules of an electronic auction, the winner of which, as a rule, is the participant the lowest bidder. Why do you need a work permit to work in electrical installations?

The main function of the document is to accurately record what kind of work certain employees are allowed to do. Here it is written:

  • safety measures that employees must apply when performing their work duties (switching off voltage and power supply, de-energizing, grounding, etc.),
  • targeted safety briefings conducted with them and other conditions,
  • persons responsible for safety.

  • an order to perform a specific task at one place or to perform several consecutive similar works at several sites;

  • intermediate work order for work on individual parts of the whole facility, its additional equipment: can only be issued as an annex to the already issued general work order.

What is piecework salary?

Piece payment allows the employer to increase the productivity of workers, it is especially relevant in production - factories, factories, etc. Unlike time-based payment, a piecework worker is interested in doing as much as possible, because the more he works, the higher his salary will be.

Simple piecework wages

– this is the result obtained by multiplying the number of products (works, services) produced by the piece rate.

For example, a worker must make 5 parts in 2 hours, which means his hourly production rate is 2.5 parts per hour (5 parts: 2 hours). At an hourly rate of, say, 250 rubles. per hour, piece rate is: 250 rub./hour: 2.5 parts = 100 rub./piece. Knowing how many parts an employee produced per month (let’s say 450 pieces), it’s easy to calculate his salary: 100 rubles/piece. x 450 pcs. = 45,000 rub.

So in the remuneration system, the piece rate determines how much a unit of work result should cost.

If employees are also paid bonuses for good results, then we are talking about piecework bonuses.

For example, in production the following prices are established: when producing 100 products per month, the piece rate is 200 rubles/piece, and products produced in excess of the norm of 100 pieces are paid at a rate of 250 rubles/piece. If an employee produced, for example, 115 products, then his salary will be calculated as follows:

(100 pcs. x 200 rub./pc.) + (15 pcs. x 250 rub./pc.) = 23,750 rub.

Who can use BSO

A work order as one of the types of BSO can be used by organizations or entrepreneurs that do not have cash register equipment and the ability to issue cash receipts. At the same time, they must be engaged in providing services to the population.

Recent changes to the legislation regulating the use of cash register equipment have significantly reduced the list of business entities that can issue work orders instead of cash receipts.

From July 1 of this year, single tax payers or entrepreneurs applying the patent taxation system who are engaged in retail trade and public catering are required to use CCT, provided that they use hired personnel.

In addition, Law No. 54-FZ (Article 2) establishes the types of services that can continue to be provided using BSO. This:

This document is subject to special accounting and storage, and it can be destroyed only after the company has been audited by the tax authorities.

Validity periods

The validity periods of permits depend on the industry and are determined by various regulations. The document is issued with a validity period necessary to fulfill the specified scope, but not more than 15 calendar days (counting from the date of issue). The official who issued the order has the right to extend it once, but for no more than 15 days. If the period has expired, a new outfit is issued (Order of the Ministry of Labor of Russia dated 03/07/2018 No. 127n, Order of the Ministry of Labor of Russia dated 03/28/2014 No. 155n).

POT RO 14000-005-98, which regulates hazardous work, allows you to issue a work order for a period of no more than 5 days. If work was interrupted, the permit is still valid provided that the conditions have not changed.
