The participation of medical workers in certification is voluntary. In the process, the specialist’s personal worth, level of knowledge, practical skills, suitability for the position held, and professionalism are assessed.
Certification of doctors for a category carries with it a certain interest:
- It's prestigious. Allows you to occupy a higher position in a medical institution, allows you to draw the attention of management to yourself. Quite often, the categories of doctors are indicated on signs at the entrance to their office.
- In some cases, the highest category allows you to reduce moral or physical responsibility to the patient’s relatives. Like, if such a highly qualified specialist could not solve the problem, then it is difficult to think what would have happened if a less experienced doctor had been in his place.
- Material side. Medical categories of doctors and promotion through the levels of the medical hierarchy allow for an increase in the basic salary.
Harmful working conditions for which additional days off are due
Harmful working conditions mean the presence of certain hazardous factors that have a negative impact on the worker’s health. Unfortunately, when working in such conditions for a long time, workers quite often develop previously absent diseases or aggravate illnesses that were practically not bothering them before. In this regard, the life expectancy of some categories of doctors is significantly reduced.
All hazardous working conditions are divided into four levels:
- First: labor activity in the presence of risk factors causes slight deviations in the employee’s health, the elimination of harmful conditions will lead to a complete restoration of well-being;
- Second: performing labor functions in hazardous conditions for more than 15 years will cause the appearance of chronic diseases;
- Third: constant interaction with risk factors causes significant deviations in the body, which can cause loss of work activity and exacerbation of serious diseases;
- Fourth: the most harmful factors, the systematic impact of which on the human body leads to a complete loss of ability to work.
According to the provisions of labor legislation, an employer whose subordinates perform work duties in the above working conditions is obliged to provide individuals with an additional vacation period, the duration of which should not be less than 7 days, and make payment for each day of rest.
Types of certifications
The legislation distinguishes several types of certification activities:
- assignment of the title “specialist” after determining theoretical and practical skills;
- qualification category of doctors (obtaining);
- category confirmation.
Determining the level of knowledge for the designation of a “specialist” is a mandatory step before appointment to the position of a doctor. Conducted by special commissions in postgraduate education institutions. The following candidates will be considered:
- after internship, master’s degree, residency, postgraduate study, if there is no diploma “doctor-specialist”;
- those who have not worked for more than 3 years in a narrow specialty;
- those who did not undergo certification in a timely manner to obtain qualifications;
- persons who are denied the opportunity to receive the second category for objective reasons.
Each doctor has the right to receive a category in several specialties at the same time, if they are related. The main requirement is work experience in the required specialization. The category of general practitioner is an exception.
How often do I need to undergo certification?
Certification of medical workers is carried out once every 5 years. The category that is assigned is valid on the territory of our country from the day when the order for its receipt is issued. Specialists have the right to strive to obtain a higher category of qualification, but only 3 years after the issuance of an administrative act on assignment. Allowances for medical categories are paid throughout the validity period.
When requalifying, the duration of work in the newly acquired specialization begins to count from the moment the work begins in the new category. Federal executive authorities create central commissions to certify doctors. Under departments, they are created by the executive branch (federal bodies), Russian academies of sciences and organizations that have subordinate pharmaceutical and medical organizations. The executive authorities of the constituent entities of the Russian Federation create territorial certification commissions.
Basic rules and requirements
There are second, first and highest categories of doctors. There is a rule of consistency in receiving, but there are exceptions. The requirements are discussed in the table.

Accreditation of medical professionals
Accreditation of a specialist is a procedure for determining the compliance of a person who has received a medical, pharmaceutical or other education with the requirements for carrying out medical activities in a certain medical specialty or pharmaceutical activity. Accreditation of a specialist is carried out by an accreditation commission upon completion of professional educational programs of medical education or pharmaceutical education at least once every five years.
In pursuance of this norm, the Ministry of Health approved:
- Regulations on the accreditation of specialists (approved by Order of the Ministry of Health of Russia dated June 2, 2016 No. 334n);
- The procedure for issuing a certificate of accreditation of a specialist, the form of a certificate of accreditation of a specialist and technical requirements for it (approved by Order of the Ministry of Health of Russia dated 06.06.2016 No. 352n).
It should be noted that Order of the Ministry of Health of Russia dated February 25, 2016 No. 127n approved the terms and stages of accreditation of specialists, as well as categories of persons with medical, pharmaceutical or other education and specialists subject to accreditation. From 02/02/2018 this Order became invalid. Instead, the Russian Ministry of Health approved new terms and stages for the accreditation of specialists on the basis of Order No. 1043n dated December 22, 2017.
Validity periods
The certification category of a doctor is valid for 5 years after signing the order for its receipt. After time has passed, the specialist must confirm the current category or receive another category.
According to the old orders, there were certain circumstances that were classified as social benefits and made it possible to extend the period of the current qualification. These included:
- pregnancy and child care under 3 years of age;
- a month after dismissal due to reduction;
- business trip;
- state of temporary disability.
The benefits are not valid at this time. The certification commission may decide to extend the validity period at the request of the head physician of the medical institution. If a doctor refuses to appear for the commission, his category is automatically removed after a five-year period from the date of assignment.
Qualification categories for teaching staff according to the new rules
On June 15, 2014, Order No. 276 of the Ministry of Education and Science of the Russian Federation of April 7, 2014 came into force, approving a new procedure for the certification of teaching staff of organizations engaged in educational activities (hereinafter referred to as the Certification Procedure) (see the blog “Certification of teaching staff - a new order” “ Certification of teachers for suitability for the position held”). The Central Council of the Trade Union of Public Education and Science Workers of the Russian Federation has prepared a “Commentary on the procedure for certification of teaching staff” (hereinafter referred to as the Comment) (see Information Bulletin of the Central Committee No. 6. Certification of teaching staff of organizations engaged in educational activities. Moscow, June 2014), in which provides explanations of certain provisions of the Certification Procedure.
About qualification categories
According to the new Certification Procedure , qualification categories previously established for teaching staff of state and municipal educational institutions are retained for the period for which they were established.
A teacher can be certified for the first and highest qualification categories. Certification for a qualification category is carried out at the personal request of the teacher.
The first qualification category for teaching staff is established on the basis of:
- stable positive results of students mastering educational programs based on the results of monitoring conducted by the organization;
- stable positive results of students mastering educational programs based on the results of monitoring the education system, carried out in the manner established by Decree of the Government of the Russian Federation of August 5, 2013 N 662 (see the blog Monitoring in Education);
- identifying the development of students’ abilities for scientific (intellectual), creative, physical education and sports activities;
- personal contribution to improving the quality of education, improving teaching and educational methods, transmitting experience of practical results of their professional activities to teaching teams, active participation in the work of methodological associations of teaching staff of the organization.
The highest qualification category for teaching staff is established on the basis of:
- students achieving positive dynamics in the results of mastering educational programs based on the results of monitoring conducted by the organization;
- students achieving positive results in mastering educational programs based on the results of monitoring the education system, carried out in the manner established by Decree of the Government of the Russian Federation of August 5, 2013 N 662;
- identifying and developing students’ abilities for scientific (intellectual), creative, physical education and sports activities, as well as their participation in olympiads, competitions, festivals, competitions;
- personal contribution to improving the quality of education, improving teaching and upbringing methods, and the productive use of new educational technologies, broadcasting the experience of the practical results of their professional activities, including experimental and innovative ones, to teaching teams;
- active participation in the work of methodological associations of teaching staff of organizations, in the development of program and methodological support for the educational process, and professional competitions.
The qualification category is established for a period of 5 years . The validity period of the qualification category cannot be extended .
Qualification categories are retained until the end of their validity period upon transfer to another organization, including one located in another subject of the Russian Federation.
The expiration of the highest qualification category does not limit the right of a teaching worker to subsequently apply to the certification commission for certification in order to establish the highest qualification category for the same position.
Who conducts the certification
Certification of teaching staff of organizations under the jurisdiction of federal executive authorities is carried out by certification commissions formed by the federal executive authorities under whose jurisdiction these organizations are.
Certification of teaching staff of organizations under the jurisdiction of a constituent entity of the Russian Federation, teaching staff of municipal and private organizations is carried out by certification commissions formed by authorized government bodies of the constituent entities of the Russian Federation.
The regulations of the certification commissions should ensure that teaching staff have the right to submit applications to the certification commission at any time , ensure that their applications are considered within no more than 30 days , and certification is carried out within a period not exceeding 60 days and taking into account the validity period of the previously established qualification category.
Application for certification
Certification of teaching staff is carried out on the basis of their applications for certification. The application for certification indicates the qualification category and position for which the teacher wishes to undergo certification.
The certification procedure does not provide for the presentation by the teaching staff, in addition to the application, of any additional information and materials necessary at the stage of carrying out a comprehensive analysis of his professional activities, as well as the appointment of any tests .
The application is submitted by the teacher directly to the certification commission , or sent to the certification commission by mail with a return receipt or notification in the form of an electronic document using public information and telecommunication networks, including the Internet.
The certification procedure does not provide for the coordination of applications of teaching staff with the employer, as well as quotas or the establishment of any ratios for the number of qualification categories established.
As noted in the Commentary, and determination of the deadline for certification of a teaching worker cannot be refused
- discrepancies between a teaching worker of higher or secondary vocational education and the area of training required for the position by the qualification characteristics (professional standard);
- expiration of the qualification category (first or highest) on the day of application;
- in passing certification for the first qualification category in case of refusal to establish the highest qualification category, including if the application for this was submitted on the day when the decision of the certification commission was made to refuse;
- being on parental leave until the child reaches the age of three;
- presence of a break in teaching activities;
- short duration of work in the organization at the new place of work.
Also, acceptance of an application and certification cannot be denied to teaching staff who, for any reason, before submitting the application, did not exercise the right to training and additional professional education at least once every three years, as provided for by the Education Law of December 29. 2012 No. 273-FZ.
At the same time, the Certification Procedure provides the grounds on which teachers may be refused to accept applications :
- if a teaching worker applies for the establishment of the highest qualification category for the first time, without having the first qualification category;
- if the application for the establishment of the highest qualification category follows earlier than two years after the establishment of the first qualification category;
- if the application for the establishment of the first or highest qualification category follows before the expiration of one year from the date the certification commission made a decision to refuse to establish the qualification category.
There are no other grounds on which teaching staff may be refused to accept an application for certification to establish a qualification category .
How is certification carried out?
from teaching staff for certification are considered by certification commissions within no more than 30 calendar days from the date of their receipt, during which:
- a specific period for certification is determined for each teaching worker individually, taking into account the validity period of the previously established qualification category;
- Teaching staff are notified in writing of the date and place of their certification.
The application is considered at a meeting of the certification commission, which is considered valid if at least two thirds of the total number of its members are present.
A teaching worker has the right to personally attend his certification at a meeting of the certification commission. If a teacher fails to appear at a meeting of the certification commission, certification is carried out in his absence.
The assessment of the professional activities of teaching staff in order to establish a qualification category is carried out by the certification commission based on the results of their work, provided that their activities are related to the relevant areas of work .
The decision by the certification commission is made in the absence of the teacher being certified by an open vote by a majority vote of the members of the certification commission present at the meeting. In case of equality of votes, the certification commission decides to establish the first (highest) qualification category. When undergoing certification, a teacher who is a member of the certification commission does not participate in voting on his candidacy.
Based on the certification results, the certification commission makes one of the following decisions:
- establish the first (highest) qualification category (indicate the position of the teaching worker for which the qualification category is established);
- refuse to establish the first (highest) qualification category (indicate the position for which the teaching worker is refused to establish a qualification category).
The results of the certification of a teacher directly present at the meeting of the certification commission are communicated to him after the voting results are summed up.
Registration of certification results
An attestation sheet is not issued during certification carried out to establish qualification categories .
The decision of the certification commission is documented in a protocol , which is signed by the chairman, deputy chairman, secretary and members of the certification commission who took part in the voting.
Based on the decisions of certification commissions on the results of certification of teaching staff, the relevant federal executive bodies or authorized government bodies of the constituent entities of the Russian Federation issue administrative acts on the establishment of teaching staff of the first or highest qualification category from the date of the decision of the certification commission, which are posted on the official websites of these bodies in Internet network.
Implementation of the decision of the certification commission
The decision of the certification commission comes into force from the date of its adoption .
When a teaching staff with the first qualification category makes a decision from the certification commission to refuse to establish the highest qualification category, he retains the first qualification category until its expiration.
Pedagogical workers who were denied a qualification category during certification shall apply, at their request, to the certification commission with an application for certification for the same qualification category no earlier than one year from the date the certification commission makes the corresponding decision.
From the date of the decision of the certification commission, teaching workers who have been assigned a qualification category have the right to an increased level of remuneration, which is one of the main tasks of certification (clause 3 of the Certification Procedure).
The certification procedure establishes that ensuring differentiation of wages for teaching staff with a qualification category is carried out taking into account the established qualification category and the volume of their teaching (pedagogical) work. Thus, it is emphasized that it is unlawful to increase wages only for the volume of teaching (pedagogical) work within the norm of hours of teaching work established for the wage rate , and also only for the main place of work.
About the appeal
The results of certification in order to establish a qualification category (first or highest), a teaching worker has the right to appeal in accordance with the legislation of the Russian Federation.
On some grounds for maintaining the level of wages for a certain period, taking into account the existing qualification category
The prohibition on extending the validity period of a qualification category, provided for in paragraph 24 of the Certification Procedure, does not mean that after its expiration, under certain circumstances, teaching staff cannot maintain the existing level of remuneration.
Here's what the Commentaries say about it.
“The Industry Agreement for 2012-2014, concluded between the All-Russian Trade Union of Education and the Ministry of Education and Science of the Russian Federation, contains a recommendation to enshrine in regional and other agreements and collective agreements the grounds when teaching staff whose qualification category has expired will have the right to maintain (establish) the level of remuneration provided for persons with the appropriate qualification categories.
For example, such grounds for the purpose of material support for teaching staff may include cases where the validity of the qualification category has expired (expires):
- while on parental leave until the child turns three years old;
- in a period of no more than one year before the day of retirement age;
- during the period of exercising powers on an exempt basis as part of an elected trade union body or within six months after their end;
- during the period of consideration by the certification commission of the teacher’s application for certification and (or) during its passage.
The listed reasons for maintaining the level of remuneration for a certain period, taking into account the existing qualification category, are not exhaustive. Regional and territorial agreements and collective agreements may provide for other grounds.
Regional and territorial agreements and collective agreements may also provide for provisions according to which teaching workers who have a qualification category for one position will be paid for performing teaching work in a position with a different name, for which a qualification category has not been established, but for which job responsibilities, curricula, work profiles (activity profiles) in an educational institution coincide.
Recommendations on the remuneration of teaching staff, taking into account the existing qualification category for performing teaching work in a position with a different name, for which a qualification category has not been established, are provided in Appendix No. 2 to the Federal Industry Agreement. The list of positions in these recommendations is not exhaustive and may be supplemented.
Regional agreements may establish provisions for certification for the same qualification category for teaching staff with honorary titles, industry insignia, state awards and other incentives received for achievements in teaching, without carrying out a comprehensive analysis of the professional activities of teaching staff.” .
Note: 1. According to the Decree of the Government of the Russian Federation of August 5, 2013 No. 662, monitoring is carried out by state authorities, executive authorities of constituent entities of the Russian Federation and local governments at least once a year in accordance with the procedures, timing and monitoring indicators established by the specified authorities. Documentation
Four months before recertification, the specialist must submit an application for confirmation or obtaining a higher qualification. Passport details, existing category and date of receipt, personal signature are indicated here.
A certification sheet and a report on the work done over the past few years, approved by the chief physician of the health care facility and the human resources department where the certified person works, are also filled out. Copies of documents on education, work history and assignment of current qualifications are also sent to the commission.
Procedure for granting leave to medical staff
The Labor Code establishes a certain procedure for providing employees of medical institutions with the required vacation period, which is subject to mandatory compliance by both the physician and his boss. The main features of going on vacation are:
- After employment in a medical institution, the employee is required to work for at least six months to obtain the opportunity to take care of the annual vacation period, subject to payment by the employer. An exception is pregnant employees who can exercise this right immediately after concluding an employment contract;
- To notify the employer of the desire to go on vacation, the health worker prepares a corresponding application. A necessary requirement, in this case, is compliance with the vacation schedule approved by the organization. The document should include the following information:
- Full name of the employer in whose name the application is being submitted;
- Full name of the employing organization;
- Full name of the applicant employee indicating the position held;
- The text part must contain a request for removal from work functions in connection with going on vacation, and it is necessary to indicate the specific period of stay on vacation. If a citizen wants to add existing additional vacation days to the mandatory four-week period, this wish must be indicated here;
- Date of document preparation;
- Personal signature of a medical professional.
- After the application is signed by the head of the medical institution, a corresponding Order is issued to send the person on leave. The applicant employee must put his own signature on this document;
- Based on the administrative document, the employee is accrued due vacation payments. Funds must be issued no later than three days before the first day of vacation.

Attestation report
The introduction includes information about the identity of the doctor and the medical institution where he holds a position. The characteristics of the department, its equipment and staff structure, and the performance indicators of the department in the form of statistical data are described.
The main part consists of the following points:
- characteristics of the population undergoing treatment in the department;
- possibility of carrying out diagnostic measures;
- carried out medical work with the indicated results for specialized diseases;
- deaths over the past 3 years and their analysis;
- implementation of innovations.
The conclusion of the report consists of summarizing the results, indicating possible problems and examples of their solutions, and opportunities for improvement. If published materials are available, a copy is attached. A list of literature used and studied over the past few years is indicated.
Duration of leave for different categories of health workers
Due to the fact that almost all areas of medicine include certain risk factors for the health of medical personnel, whose representatives systematically interact with various toxic drugs or irradiating equipment, the legislator has granted these categories of workers the right to receive additional days off. Each specialist receives points that are used in making decisions about qualifications. They are awarded for attending conferences, including international congresses, giving lectures to colleagues or nursing staff, distance learning with a final certificate, and taking courses.
Additional points are awarded for the following achievements:
- publishing of textbooks, manuals, monographs;
- publication of an article;
- obtaining a patent for an invention;
- presentation at symposiums with a report;
- speaking in institutions and the media;
- obtaining a title;
- defense of the thesis;
- awards by public authorities.
Changes in the system of admission to medical practice
In accordance with Art.
100 of the Federal Law of November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in the Russian Federation” (hereinafter referred to as Law No. 323-FZ), until January 1, 2026, persons who have received a medical education have the right to carry out medical activities in the Russian Federation and having a specialist certificate. However, on January 1, 2020, Art. 69 of the specified Federal Law, according to which the specialist’s certificate is replaced with a certificate of accreditation of a medical worker. Moreover, persons with medical education and others will have the right to carry out medical activities.

At the same time, the legislator has provided for a transition period, namely, in accordance with Part 1.1 of Law No. 323-FZ, the transition to the accreditation procedure for specialists is carried out in stages from January 1, 2020 to December 31, 2025 inclusive. It is worth noting that such a transition period was not previously provided for. The changes were made by Federal Law No. 389-FZ of December 29, 2015 “On Amendments to Certain Legislative Acts of the Russian Federation.”
In addition, Part 2 of Art. 100 of Law No. 323-FZ, according to which specialist certificates issued to medical and pharmaceutical workers before January 1, 2021 are valid until the expiration of the period specified therein (previously, this provision applied to specialist certificates issued before January 1, 2020).
Composition of the commission
The commission consists of a committee, whose work takes place during breaks between meetings, and a narrowly focused expert group, which directly conducts certification of a specialist (exam, testing). Both the committee and the expert group consist of persons holding the following positions:
- The chairman, who oversees the work and divides responsibilities between the members of the commission.
- The deputy chairman performs the functions of the chairman in full in his absence.
- The secretary is responsible for registering incoming documents, preparing materials for the work of the commission, and recording decisions.
- The deputy secretary replaces the secretary and performs his duties during absence.
Each expert group includes specialists from related specialties. For example, the category of dentist and its receipt/confirmation requires being in the group of a periodontist, orthodontist, pediatric dentist, or therapist.
How to get
In medicine, there are types of certifications:
- obtaining the title of “specialist” as a result of assessing knowledge in theory and practice;
- assignment of the appropriate qualification category, which is possible only after certification;
- her confirmation.
Consideration of candidates for appointment as “specialists” is carried out in institutions of postgraduate education. This is a mandatory procedure for taking up the position of a doctor.
The event is attended by students after master's degree, internship, postgraduate school, residency, if there is no certificate of “doctor-specialist”, persons who have not worked for more than 3 years in their specialty, who were not awarded a qualification within the required time frame or were denied it.
Certification of a doctor, nurse, ambulance paramedic, and so on allows you to obtain a qualification category and is considered by the state as a mechanism for monitoring the quality of personnel training and services in this area. This event is considered voluntary.
Certification and recertification evaluates the competence of a medical worker and his compliance with the qualification characteristics of the position.
General list of requirements for obtaining categories 1 and 2, in addition to length of service:
- knowledge of theory;
- confident practical skills;
- application of modern methods and techniques for diagnosis, treatment, prevention, and rehabilitation of patients;
- ability to use health-improving diagnostic equipment in the practiced area, analyze qualitative and quantitative performance indicators, and draw up reports;
- educational activities, improvement of practical skills, and so on.
General requirements for candidates for the highest category, in addition to work experience:
- high level of training in theory and practice;
- knowledge of interrelated disciplines;
- application of the latest methods of diagnosis, treatment, prevention, rehabilitation;
- use of modern health-improving diagnostic equipment in the field of practice;
- in-depth analysis of qualitative and quantitative indicators in work, drawing up reports;
- educational activities, improving one’s own skills and abilities, and so on.

Order of the meeting
Certification is scheduled no later than three months from the date of receipt of data about the specialist by the committee. If the data does not match the requirements for the latter, the documentation will be refused (no later than 2 weeks from the date of receipt). The secretary of the committee agrees with the chairman of the expert group of the required specialization on the date of the examination.
Members of the expert group review the certification works of doctors for the category, completing a review for each of them, displaying the following data:
- level of practical skills of a specialist;
- participation in social projects related to the medical field;
- availability of published materials;
- self-education of the certified person;
- compliance of knowledge and skills with the declared category of doctors.
The examination must take place within two weeks from the date of receipt of the report. The result of the review is an indicator of the possible outcome of the certification. The secretary informs the specialist of the date of the meeting, which includes an interview and testing. More than 70% of correct answers allow you to consider the test passed. The interview takes place by questioning the person being certified according to theory and practice, the knowledge of which must correspond to the requested qualification.
The meeting is accompanied by the preparation of a protocol, which is signed by the members of the expert group and the chairman. The final decision is noted on the qualification sheet. A specialist receives the right to retake the exam only after a year. Within 7 days, the certified person receives a document confirming the promotion, reduction or refusal to assign a category.
Conditions for refusal to accept documents by the commission
Documents may not be accepted in the following cases:
- if there are no necessary papers that are needed for the certification commission to analyze the issue of awarding a specialist with a qualification category;
