Why do we need a unified T-2 form?
Form T-2 was approved by Decree of the State Statistics Committee of the Russian Federation dated January 5, 2004 No. 1 and is an employee’s personal card, which contains information about who he is, when he was born, where he lives, and also reflects his marital status, information about the level of education and professional seniority The specified data is entered based on:
- passport of a citizen of the Russian Federation (or other document confirming identity);
- work book (employment contracts);
- SNILS;
- documents confirming secondary specialized or higher education;
- military ID;
- other documents: TIN certificate, certificates and diplomas of advanced training, retraining and the presence of special skills and knowledge.
Since 2013, the use of unified forms has ceased to be mandatory, so the employer can develop a personal card form independently. But most of them prefer to use the T-2 form, which provides for the presence of fields for all the necessary data about the employee.

This document is drawn up for each employee of the company, while the individual entrepreneur is not assigned the obligation to maintain this form. Filling out the form is the responsibility of personnel services employees or authorized persons. As information about an employee changes, adjustments are made to the card based on supporting documents, for example:
- about a change of surname - on the basis of a marriage certificate and a Russian passport;
- change of position - on the basis of a transfer order;
- dismissal of an employee - on the basis of a dismissal order;
- obtaining a new specialty - in accordance with the diploma of education.
After an employee’s dismissal, the card remains with the company, because its safety must be ensured by the employer for 75 years from the date of registration.

Filling out the header and entering general data
The format of the fields that the sample card contains is considered accessible, but the confidentiality of certain information requires encoding. In general, you need to fill out the form with a black or blue pen, but until some information is clarified or if it is possible to change it, temporary recording in pencil is allowed.
In specially designated fields, the full name of the organization and its codification according to OKPO and OKUD are written down, as well as the employee’s personnel number, TIN certificate number, SNILS and the nature of the work (permanent or temporary). Personal data is taken from the passport. Information about language proficiency is presented in one of three types:
- owns;
- explained;
- translates with a dictionary.

Reserve fields are provided for information about the education received. They allow changes to be made if a citizen receives an additional specialty. If there are several professions, only the one for which the employee was accepted into the organization is indicated. Please note that when filling out your passport information you will have to indicate your registration address and actual residence. In general, these data do not coincide.
Unified form T-2 for VUR
The T-2 card is also necessary for registering VUR (military registration workers). According to the Law “On Military Duty and Military Service” dated March 6, 1998 No. 53-FZ, the employer must maintain military records, which are carried out on the basis of employees’ military ID cards. Previously, there was a T-2 VUR form, which had a shortened T-2 appearance.
This form consisted of 3 sections:
- Section 1 - general information about the employee;
- Section 2 - information about the employee’s military duties;
- Section 3 - information about hiring and transfers.

Entering information about the VU
We gradually approached the topic that interests us. The basis for entering information into a personal card is a military ID or registration certificate. The procedure for filling out some fields depends on the ranks of those liable for military service and types of military registration.
Entering information about reserve officers
The first paragraph in the form is not filled out, but the military rank is entered immediately. The data is taken from the military ID. When assigning titles sequentially, you need to write the latest date. The name of the profile is indicated in paragraph 7 of the military ID. For most officers this is a command profile, but there may be medical, pedagogical, legal, etc.
A citizen's military specialty is coded with a six-digit number. The code from the military ID is entered in the VUS column. The lack of a specialty also has a designation. Sometimes the accounting group is also indicated. For the Russian army, this is the abbreviation “RA”.
Point No. 5, indicating the fitness category, must contain a letter corresponding to the category that was assigned based on the results of the meeting of the military commission. If there is no category in the document, then the letter “A” is entered on the card.
The following entry is made if there is a mobilization order. This is a document issued by the military registration and enlistment office for data verification. It involves appearing without a summons during the announcement of mobilization. The team number taken from the instructions is entered into the form. When registering a person for special registration, the number of the deferment certificate is entered in the appropriate field. Data on the VU often changes, so it is recommended to write it in pencil.

About the privates
The reserve category is an indicator that determines military service. If a citizen has served in the Armed Forces for at least a year, then he is in the 1st category in the reserve. Persons who have not served have category 2. We will skip the items that do not cause difficulties to fill out or are carried out by analogy with the procedure for entering information about officers discussed earlier and highlight only those that have features. Opposite the “profile” column, without abbreviations, it should be written: “soldiers” or “sailors.”
All other fields are filled out according to the military ID data without any additions. It should be noted that this method of maintaining a personal card is typical not only for privates, but also for sailors, warrant officers, midshipmen and sergeants.
About conscripts
Personal forms for citizens who are to be drafted into the Armed Forces are filled out on a general basis. A conscript is considered a citizen whose upper age limit has not reached 27 years. Employment for this category of citizens is often found if a deferment from the army was received.
The first field, responsible for the stock category, is not filled in for obvious reasons. If an employee intends to work in an organization for a long time, then the entry can be made in pen, since after he reaches the age of 27 years, he will receive a military ID and will be considered a reserve soldier with category 2.
Instead of military rank, you should write “Subject to conscription.” It often happens that after service a citizen returns to his previous place of work. To avoid unnecessary corrections, fields with changing information that do not allow additions should be filled out in pencil. After removing the card from the file cabinet, be sure to duplicate all entries with a pen.
Another distinctive feature of filling out a personal card for a conscript is that point No. 7 is not filled out.
All completed information is agreed upon with the employee himself. After reviewing it, he must sign the document. By the way, he must set his signature when making any changes. An employee is subject to deregistration if he has reached a certain age stipulated by law or is leaving for another place of residence. Before the personal card is removed from the card index, it is necessary to make a note in paragraph 8 at the military registration and enlistment office indicating deregistration.

The most important requirement for filling out forms, which is presented to the person responsible for accounting work, is accuracy. The fact is that, at the request of some military registration and enlistment offices, forms with corrections can be equated to violations in the conduct of military personnel. It is impossible to challenge their claim, so before entering any information, you should check its accuracy several times. Handwriting must be calligraphic, and entries must be made in strict accordance with the rules.

What is a personal card used for?
The applicant was interviewed, hired, an acceptance order was issued, and now he needs to go through the employment procedure. To do this, he needs to submit a set package of documents containing a lot of heterogeneous information.
To systematize this information for further accounting, the data is recorded on specially designed cards. Thus, information about all employees of the enterprise forms a file cabinet convenient for complete accounting.
The State Statistics Committee of the Russian Federation developed and approved in Resolution No. 1 of January 5, 2004 “On approval of unified forms of primary accounting documentation for recording labor and its payment” a form for filling out a personal card, called T-2.
On such a form it is convenient to reflect all significant information about the employee. If the enterprise considers it necessary, it can develop its own personnel data register on its basis.
We organize military registration
A personal account card is created at work immediately after the order for employment is issued.
At the same time, you need to make an entry in the employee’s work book about hiring.
A personnel number is an employee number that is assigned to him at a given enterprise for internal accounting. When filling out a personal card, a personnel number has already been assigned to him (from the manager’s order). TIN (Taxpayer Identification Number). An employee may not provide a TIN when applying for a job, because this is not specified either in the Labor Code or in the Tax Code, and, accordingly, is not mandatory. If the employee does not have a TIN or has not provided it, this field is not filled in. SNILS (state pension insurance certificate number).
Based on Part 1 of Art. 65 of the Labor Code of the Russian Federation, the employee is obliged to provide the employer with SNILS.
New accounting documents
Officials excluded from the legislation the obligation of employers to use personal cards of employees in form No. T-2 for maintaining military records. But this news is pleasant for personnel officers only at first glance. Instead of the abolished personal cards, other accounting documents will be introduced. Which? So far, the Russian Ministry of Defense authorized for this has not yet approved the list, forms, procedure for storing and filling out such special documents. And even in paragraph 23 of the Methodological Recommendations of the General Staff of the Armed Forces of the Russian Federation on maintaining military records in organizations it is stated that organizations must maintain personal cards of employees in Form No. T-2. In this case, the resolution of the Government of the Russian Federation is more important, so you can refuse form No. T-2 and wait for further instructions and forms from the Ministry of Defense.
Questionnaire for a citizen
The application form for a citizen to the military registration and enlistment office (Form 9), provided for in Appendix 4 to the Instructions, approved by Order of the Ministry of Defense No. 400 of October 2, 2007, is filled out by school directors and heads of other educational institutions. The questions concern persons who are subject to initial registration at 16-17 years of age. Such a form should not be filled out by future conscripts; this should only be done by the head of the institution.
This form contains a list of questions regarding the following information:
- the conscript’s passion for disciplines, what clubs and sections he attends;
- success in the educational process, attitude to responsibilities;
- certain abilities, relationships in a team, level of sociability, reaction to criticism;
- family relationships, cases of seeking medical help, alcohol or drug use.
These data are subsequently recorded on the conscript’s study sheet and help determine the area of interests and hobbies of the future conscript.
